Collective Impact

Collective Impact brings people together, in a structured way, to achieve social change.

 

Along with Northeastern Vermont Regional Hospital, we guide the work of The collective impactCollective Impact Project in Caledonia and southern Essex counties, an anti-poverty project of the Accountable Health Community.

The Collective Impact Project did not create a new anti-poverty program, but we agreed on a common set of goals, outcomes, and success indicators.  We align resources, analyze and track progress, and coordinate practices to what has proven to work for families, children, and individuals in our region.

With a common agenda and shared tools for tracking progress, we coordinate our collective efforts, based on relationships of trust among participants.  No more piecemeal approaches; no more siloes of programs and systems. We work together for systemic change.

The Collective Impact Project of the Accountable Health Community is one of five initiatives across the US sponsored by Feeding America.
